APPLICATION TO ADOPT AN
AFGHAN HOUND
Thank you for your interest in adopting a
rescued Afghan Hound. Please help us understand what qualities you are
looking for in an Afghan Hound, and what your lifestyle and needs are by
completing application.
To save this file to
your computer, go to “edit’ on your menu bar, select all, (document will
highlight itself), copy and paste into a word processing document and save to
your computer. You should then be able to open the
document using your own software allowing you to type in your information.
Once completed, save document to your hard drive, then either print and mail via

After
we review your completed application, and check references provided, we will
schedule a time to meet with you in your home. All rescued Afghan Hounds are
seen by a veterinarian for a physical exam, vaccinations, heartworm check, and
stool check, are placed on heartworm preventative and are spayed/neutered prior
to placement. Rescued dogs are not placed with AKC registration papers, but we
will assist owners in obtaining an “ILP number” through AKC, which allows dogs
to compete in obedience, agility, and field trials. A adoption fee is required
for each dog, at the time of placement. You do not need to send a check with
this application.

below, I also authorize representatives of Afghan Hound Rescue to contact the

Humane Society, animal welfare/control or rescue organization to release any
information that they may have on file to me to representatives of Afghan Hound
Rescue. Should Afghan Hound Rescue receive any negative information about me
from any animal welfare organization, they will inform me. I also authorize
